New and Established Patients

When scheduling your appointment:

Please have all insurance information ready and confirm with your insurance company that we are in-network with your specific plan. We will ask for your insurance card with each visit.

We have automatic reminders for your appointment by email, telephone, and text beginning 1 week prior to your appointment. We understand your appointments may be booked several months in advance and we want to help ensure you do not miss your appointments.

All appointments, including new patient appointments, are subject to a $75.00 no show fee if a patient does not call 24 hours ahead to cancel or reschedule the appointment, or if you are more than 10 minutes late to your scheduled appointment time.

New Patients:

After scheduling your new patient appointment, please complete the medical records release form as soon as possible (at LEAST one month before your appointment is strongly advised). Obtaining records from other practices may take anywhere from weeks to months. Starting this process early will help ensure that your previous medical records are received prior to your first appointment.

A new patient appointment requires a 72 business hour verbal confirmation prior to your visit, our office will remove you from our schedule the prior day if not confirmed.

Please arrive 10-15 minutes early to ensure we have all correct information on file.

Bring all completed paperwork (if not already completed via the patient portal), insurance card, driver's license or valid photo ID, previous medical records, and a list of all current prescription medications.

Whomever you have scheduled with is who you will be establishing as your primary care provider. We do not allow patients to be seen by multiple providers unless your visit is for an acute illness. This helps the office establish consistent quality care for our patients.

We typically do not do physicals on your first appointment. This appointment is to establish care with your new provider.
