Please call our main number at 303.680.9150, press option 1 and our receptionist will be happy to make an appointment for you.

Masks are required for sick patient appointments to ensure the protection of patients and staff.

Co-payment is due at time of your appointment. Smoky Hill Family Medicine has discounted payment options for our non-insured patients.

All balances are due at time of service and if not paid, will be subjected to additional fees.

Please be on time for your appointment. If you are unable to make your scheduled appointment, we require a 24 hour cancellation notice.

Patient's who do not call or show up to an appointment will be subject to a $75.00 no show fee.

We do not accept walk-ins, appointments are required.
